REQUEST: Help about entrepreneurial networks

Dear all,

I am currently conducting research on the development of entrepreneurial networks and would like to ask if anyone knows some good articles/references for the following:

1) Questionnaires, measures, and/or scales used in surveys for networks constructs like: network content, network centrality, tie strength, network management
2) Questionnaires, measures, and/or scales used in surveys for entrepreneurial orientation constructs: innovativeness, proactiveness, risk-taking, autonomy, aggressiveness

The types of references I am looking for are articles that include the actual questions used in surveys, (for example likert-scale type questions, such as in Covin & Slevin 1990, Journal of Business Venturing) 

There seems to be a substantial amount of references for 2) above, but I have been very unsuccessful in finding similar articles for network constructs as in 1) above. 

Can anyone can help?
